Police Clearance Application Form: How to Fill It In Online

The police clearance application form lives inside your account on the PNP’s portal. For the national clearance, there’s no paper or PDF form: you fill in a sign-up form, your profile and a short booking page, and those details print on your clearance. It takes about 15 minutes with your ID beside you.
We checked each form section on pnpclearance.ph on 29 October 2026.
| Item | Answer |

What you need to fill up the police clearance form
Gather these before you sign in, because the portal can time out while you hunt for details:
- The same valid ID you plan to present on your visit
- Your complete address: barangay, city or municipality, and province
- Your birth date and birthplace
- Your height and weight
- Your father’s and mother’s full names, including your mother’s maiden name
- Your spouse’s name, if you’re married
- An email address and a mobile number you actually use
How to fill up the police clearance form online
If you’re wondering how to fill up police clearance online, know that the online application form for police clearance comes in three parts: registration, the profile, and the clearance application. Here’s the order.

Step 1: Fill in the registration form
Click Register on the portal and accept the terms. Then enter your name, gender, birth date, email and a password. Type your name the way your ID shows it, with your full middle name. Finally, open the activation email so you can sign in.
Step 2: Complete your personal information
Sign in and open Edit Profile. Then fill in your nationality, place of birth, civil status, address and contact number. Write the address in full rather than in short forms, since it prints exactly as typed.
Step 3: Add your physical details
Next, the police clearance form asks for your height, weight and complexion, plus any identifying marks such as a scar or a tattoo. Use your current height and weight. These fields confuse many people, so our guide to complexion and the other tricky fields explains what to put in each.
Step 4: Enter your family background
Add your parents’ names, and also your spouse’s name if the form asks for it. Use full names, not nicknames. If your birth certificate shows your mother’s maiden name, copy it from there so the spelling matches.
Step 5: Fill in the clearance application form
Save your profile, then click Clearance Application. Choose your purpose, a police station, a date and a session. Pick the first-time jobseeker purpose only if you have the barangay certification for it. Then select LandBank as the payment option and save.
Step 6: Review before you pay
Read the summary screen line by line. Check the spelling of every name, the purpose, the station and the date. When it all matches, save the booking, note the reference number, and then move on to payment.
Police clearance form PDF and download questions
Many people search for a police clearance application form download, expecting a sheet to print and fill by hand. For the national clearance, it doesn’t exist. The form is the online profile itself, and the PNP’s how-to-apply guide is a set of instructions, not a blank form.
So if a site offers a police clearance form PDF or a police clearance blank form for the national clearance, skip it. Some towns still use a paper form for their local clearance, but the station hands that out itself.
Police clearance form cost and time
Filling in the form is free. The money comes later, when you book: ₱150, along with a modest fee set by the payment channel you pick. As for time, plan on about 15 minutes for the profile and a few more for the booking. After that, your profile stays saved for every future application, so renewals go much faster.

Filling in the form on a phone
Most people complete the form on a phone, and that works fine. Still, a few habits help. First, turn your screen sideways on wide pages, so no field hides off the edge. Next, type names by hand instead of letting autofill guess, because autofill loves nicknames. Also, don’t paste details from a chat app, since hidden spaces can slip into a field. Finally, finish in one sitting, so the session doesn’t expire halfway.
Police clearance form mistakes to avoid
These slips show up on printed clearances all the time:
- A nickname instead of your legal name. The name must match your ID.
- A missing middle name. If your ID shows one, add it.
- Jr. or III typed into the first-name box. Use the qualifier field for suffixes.
- A short-form address. “Brgy.” and “Pob.” can print awkwardly, so spell things out.
- The wrong purpose. The purpose prints on the clearance, and the fee isn’t refunded.
- Saving without a final read. A printed clearance can’t be corrected.
